Online Application
The eApp (preferred)- filing online is faster and easier! If you file online, you must also COMPLETE the eApp online Activities Chart, the four required short (150 word) Personal Statements, and upload scanned transcripts or electronically transmitted transcripts and other required documents specified by the scholarship, unless otherwise indicated by specific scholarships. Click here if you must mail or hand deliver required documents to OSAC. |
Paper Application Packet
The paper version is available upon request by clicking here. Consider filing the eApp online. OSAC saves your eApp online so you can update it and apply next year, allow you to track the status of your scholarships, accept awards, and update personal information on your application. |
Adobe Acrobat Reader is required to view or print .pdf (portable document format) documents. OSAC uses .pdf to distribute various forms to our customers. You need to have the appropriate version for your operating system and machine, which isn't necessarily the most recent. Click on the icon to load or upgrade to the appropriate version.
