Submitting your OSAC 2010-11 Scholarship Application

The information you submit on your OSAC scholarship application helps us determine your eligibility for specific scholarship programs.

Your complete application packet will consist of:

Electronic filers of the eApp: Your eApp online form (includes Activities Chart, required four short Personal Statements, additional scholarship essays, information, and certification, and uploaded transcripts)
Paper applicants: Your signed application form (Paper applicants, only) and the materials listed here:
- Activities Chart (available online as part of your eApp or as a separate template)
- Four Short Personal Statements (available as a separate template)
- Appropriate transcripts
- Other required documents (with the exception of additional scholarship essays, submit other required documents after receiving notification from OSAC. e.g., birth certificate, military papers)

Note: Paper applicants, make sure to write your legal name and the last four digits of your social security number (SSN) on the top of each page of the documents you enclose with your application (Activities Chart, required four Personal Statements, and other required documents).

Your eApp or paper application and required documents must arrive at OSAC by the appropriate deadline:

Tuesday, February 16: EARLY BIRD Review

Monday, March 1: FINAL Deadline. No Exceptions

If you need to make changes to your application after you have mailed or hand-delivered it, click here.

If you need to know that OSAC received your scholarship materials, use a delivery service that gives you a return receipt. Because of the volume of materials that OSAC receives, we are unable to provide you with this information.
